Information: 

   -   Full Name, Address, Nationality of the Applicant;

   -   Full Name, Nationality, Date of Birth, and Address of the Author and the Owner of work;

   -   Title, type of the work, date, place and form of publication of the work (if any);

   -   Brief description of the work.

Documents:

   -   A Notarized Power of Attorney signed by the applicant (s) (with full name capacity of the    subscriber in the case of legal  entity) (our  form  is  enclosed).

   -   Three complete copies of the work (required at filling);

   -   Notarized Deed of Assignment from author (s) to the applicant, where the applicant is not the author
